Financial / Registration Information

Pre-Registration:

If your service is scheduled in advance, we will attempt to contact you to verify and update any data such as address, place of employment, emergency contact, insurance etc.  We will also inform you of any copayments, deductibles, coinsurance amounts due at time of service.

If you would like to schedule / pre-register your appointment, please call 813-783-TEST (8378) Monday - Friday 8:00am to 5:00pm.


Registration: 

• If we were unable to pre-register you, we will confirm your demographic and insurance information upon check-in
• Verify that pre-certification/authorization has been obtained
• Obtain Photo ID and Insurance card(s)
• Collect any monies due
• Obtain consent for treatment and authorization to bill your insurance carrier

Insurance:

Your insurance coverage is a contract between you and your insurance company.  Please know what your insurance covers and what your financial responsibility is. 

There are certain insurances which Florida Hospital is non-contracted and/or out of network with.  This may result in your having to pay higher out of pocket expenses and/or possible denial from your insurance company if authorization is not obtained.  See Precertication section below.

Copay, deductible and coinsurance amounts are due at time of service.  Payments can be made via cash, checks and credit cards.  We accept American Express, Discover, Visa and Master Card.  If payment cannot be made, elective services will be cancelled and rescheduled for a time when the patient can pay.  

For Emergency Room patients, these amounts will be collected after you have seen a physician. 
If your insurance carrier is not contracted with Florida Hospital, your financial responsibility may be significantly higher. 

International/Travel Insurance:

A valid United States claims mailing address and telephone number is required when billing international carriers.  A claim number is also required for billing.  Certain travel insurance companies require that you notify them when receiving any hospital services.  Precert/Authorization may be required in many cases.

Automobile and Worker's Compensation Coverage:

A valid claim number, telephone number and claims mailing address is required when billing Auto and Work-Comp claims.  Please provide this information at time of Pre-Registration or check-in.

Medicare/Medicaid:

Florida Hospital is a Medicare/Medicaid provider. Medicare and Medicaid patients must present their current cards at the time of service. Medicare patients who do not have supplemental coverage are required to pay their deductible at the time of admission.
Medicaid eligibility will be verified.

Self-Pay:

For patients without insurance coverage, we offer the following discounts for prompt pay:

50% off total charges if paid at the time of service
40% off total charges if paid within 30 days from the date of service
30% off total charges if paid within 60 days from the date of service
20% off total charges if paid within 90 days from the date of service
 
We offer a variety of payment plans, including payment arrangements and the Hospital Expense Loan Program (H.E.L.P.) which extends payments over one to three years.

Patients who have services rendered in the Emergency Room or are admitted to the hospital receive an automatic 30% discount at the time of billing. 

Those who receive elective outpatient services are required to pay prior to services being rendered.  If payment cannot be made, the service will be cancelled and rescheduled for a time when the patient can pay.  

If you meet the criteria for Medicaid, a representative from our Medicaid eligibility vendor will assist you with the application process. 

Financial assistance may be available to patients receiving non-elective hospital services who are not covered by any form of insurance or governmental program.  Verification of income and financial information will be required.  Please call Patient Financial Services Customer Service at 813/783-6102 for information.


Price Estimates:

Florida Hospital, as a licensed healthcare facility in the State of Florida, gives notice, pursuant to Chapter 395.301 Florida Statues, that it is required, prior to provision of non-emergency medical services, to give its patients written, good-faith estimates of the reasonably anticipated charges for their treatment.  The estimates must be provided within seven days after receipt of written requests be the patients or their guardians.  Patients are also entitled to notification of revisions to the estimates.

The estimates may be the average charges for the subject procedures, and the actual charges may exceed the estimates.

Pre-Certification:

If your insurance requires a precert/referral/authorization, your PCP (Primary Care Physician) or Specialist may be required to submit clinical information to your insurance company in order to obtain the authorization for Florida Hospital. If you have any questions regarding precert/referral/authorization, please let us know. If we are unable to obtain a precert/referral/authorization, we may need to reschedule your service(s).  Authorizations are required prior to receiving any outpatient testing at Florida Hospital.

Your insurance coverage is a contract between you and your insurance company. Benefits will vary depending upon the type of insurance policy you carry.

Physician Billing:

You should expect to receive a separate bill from the physician that may assist in determining results of your service(s).
Physicians are not employees or agents of Florida Hospital. They are independent, private practicing physicians and may be individually contracted with an HMO or PPO. These contracts could be different from contracts Florida Hospital holds.
Contact your insurance company to verify that both the Hospital and the physician is contracted with your Insurance Provider Network. You should expect to receive a separate bill from your physician(s) listed below, if applicable. If you have any questions regarding bills from them, please contact them directly.

• Emergency Physicians will bill you for any services you may have received in the Emergency  Department.
• Radiologists will bill you for reading any X-rays that were taken while at the hospital.
• Consulting Physicians may be asked by your treating physician to review your care for any special needs. You will receive a bill from these physicians as well. If you have any questions about why they are billing you, please speak with your physician.
• Pathologists will bill you for any laboratory and pathology test(s) they have read while you were at the hospital.
• Anesthesiologists will bill you for services received during any surgical procedures you may have undergone while at the hospital.

Valuables:

Neither Florida Hospital nor its employees accept responsibility for the personal belongings (property) of the patient. We ask that you leave your property with person(s) other than Florida Hospital and it’s employees. If you do choose to keep your property, you cannot hold Florida Hospital or its employees responsible while it is in your possession or during any time when it is not in your possession.
Florida Hospital assumes NO responsibility for lost or damaged valuables. Therefore, we ask that other than sufficient funds to meet your co-payment/deposit requirements, you leave all other valuables at home.
If this is an unscheduled stay, please send ALL items home immediately with a family member to ensure their safety. Florida Hospital does not reimburse for lost, stolen or damaged items.
