Thank you for taking the Personal Wellness Profile (PWP).  This is the place to take the PWP to qualify for benefits and / or the L.I.F.E. Program.

1.  You will be directed to the "End User Agreement".  Select "I Agree" to continue. 

2.  Then you should see a prompt for your FHZ ID and password.  Do not enter them here unless you are a returning user. Click on "New User?".

3.  On the next screen, enter your 9-digit FHZ Employee ID number (from your payroll check, your benefit enrollment letter or ask your director for the ID number).  Do not enter the number after the dash.  Complete the entire PWP and print it if your are enrolling in the L.I.F.E. Program.

If you are enrolling a dependent, please e-mail ethan.bird@ahss.org for their ID number.
